How it works

Fill a form in on screen or print it, sign it in front of a Florida notary (in person by appointment, or online from anywhere), then have it apostilled if it is going to another country. Each form page explains the fields, the signing rules and the country-specific points.

Three steps

1. Fill it in
On screen in any PDF reader, or print and write in ink. Leave the signature and the notarial certificate blank.
2. Sign before a Florida notary
By appointment at one of our offices, or by video with a Florida online notary from any state or country. Bring a government-issued photo ID.
3. Apostille it
If the document is going abroad, we file the apostille in person in Tallahassee and return it in 5–7 business days — $220 per document, U.S. shipping included.

Why these forms

A notarized document is apostilled on the strength of the notary's certificate, not its content. If the certificate is missing the county, the date, the physical-presence or online checkbox, the ID line or the seal, the Florida Department of State sends it back — and if it slips through, the foreign registry refuses it. These forms carry the Florida statutory certificate (Florida Statutes 117.05) printed under the signature line, in the exact wording, so any Florida notary can complete it without attaching a loose certificate.

They are general templates and not legal advice. My Florida Apostille is not a law firm and cannot tell you which form to use or how to complete it; requirements differ by country and institution.
